It combines personalized in-home assistance from trained Health Ambassadors with advanced virtual clinical oversight and smart passive monitoring technologies. A tailored AI algorithm dynamically matches patients with the most suitable Health Ambassador, while integrated devices facilitate communication and enable fall detection, medication reminders, and continuous health monitoring. All collected data is consolidated within an AI-driven dashboard that promptly alerts clinicians when intervention is required, thereby elevating safety and quality of care for rural residents
