- Autonomy and Independence, Cognitive Health and Dementia, Healthy Lifestyles and Wellness, Heathcare and Health Service Delivery, Staying Connected
The TAKECARE project aims to demonstrate the adaptability and applicability of its technology in autonomous, semi-autonomous, medicalized and specialized residences for older adults. Designed to meet the diverse needs of users, families and caregivers, the solution optimizes care and follow-up management, improves residents’ quality of life and promotes a safer environment.
TAKECARE enables care to be tailored to actual needs and specific risks through intelligent monitoring and predictive analysis, reinforcing older adults’ autonomy and sense of security while facilitating their self-management. By improving communication between residents, families and healthcare professionals, the solution contributes to better care coordination and a reduction in avoidable hospitalizations, thus easing pressure on care teams and optimizing facilities’ resources.
Faced with the challenges of an aging population and the increasing prevalence of complex chronic diseases, TAKECARE integrates seamlessly into existing clinical environments, thanks to its interoperability with care management and medical records platforms. Its deployment aims to transform residential practices by facilitating proactive, personalized care, while improving organizational efficiency.
Developed by Solutions Greybox, this project is based on a model focused on innovation, prevention and personalized care.
